Once you’ve submitted the rosette request form below, you will receive an auto generated email confirmation. It is VERY IMPORTANT to retain that email confirmation as it contains a link to submit the rosette winner’s information for our online Rosette Gallery.
You should receive your rosette within two weeks. When you receive the Rosette, please send an email to Ana Dailey, CQA/ACC Administrative Assistant, to acknowledge receipt.

Remember to take a photo of the winner with the quilt! The photo of the winning quilt and quilter should be a jpg of about 2 MB.
